Long COVID — Gut, Dysautonomia and POTS
The most common post-viral pattern is not one disease but a set of systems that failed to return to baseline together: the gut barrier and its ecology, the autonomic nervous system, mast cells, and cellular energy production. Why the gut is usually involved
SARS-CoV-2 infects intestinal epithelium directly and reshapes the luminal environment. Where epithelial energy metabolism is disturbed, luminal oxygen rises, obligate anaerobes contract, and facultative organisms expand. The resulting ecology sustains barrier permeability and immune signalling long after viral clearance, which is why post-viral gut symptoms often outlast every other complaint.
Orthostatic intolerance as a downstream reading
POTS after infection is rarely a single lesion. Reported contributors include reduced plasma volume, small-fibre involvement, autoantibodies against adrenergic and muscarinic receptors, deconditioning, and mast cell-driven vasodilation. Stratifying which of these is dominant matters more for management than the label itself, and that stratification is a clinician's call.
Why energy does not return with rest alone
Where mitochondrial function is impaired, rest restores neither substrate handling nor redox balance. Post-exertional worsening is the observable signature of a system operating without reserve. Recovery sequencing — capacity before load — is the practical consequence, and it is the opposite of graded escalation applied indiscriminately.

Why do Long COVID gut symptoms persist after the infection clears?
