What it measures
Exhaled hydrogen and methane concentrations over time after a defined carbohydrate substrate, reflecting fermentation by gas-producing organisms accessible to that substrate.
Compartment sampled: Exhaled breath (end-alveolar air), sampled at intervals after an oral substrate.
Assay principle: Gas chromatography or electrochemical sensing of hydrogen and methane concentrations in parts per million, plotted as a time course against the substrate dose.
What it does not measure
It does not count small-intestinal bacteria, identify which organisms are present, locate where fermentation occurred, or establish that fermentation is causing symptoms.
Sources of variance
Each of the following can change the reported number without any change in the underlying biology.
- Orocaecal transit time
- Substrate choice (lactulose versus glucose)
- Preparation diet and smoking
- Hydrogen-consuming organisms that mask production
The common misinterpretation
Treating a positive result as a bacterial count, or a negative result as proof that fermentation is not contributing.
What one result cannot establish
- Which organisms produced the gas, or where in the bowel they were located.
- Whether fermentation observed during the test is related to the symptoms the person reports.
- Whether a rise reflects small-intestinal fermentation rather than early arrival of substrate in the colon.
What repeated results would mean
- A repeated test performed with the same substrate and preparation can describe whether a gas pattern is stable or variable over time.
- Repetition does not convert a gas curve into a bacterial count, and it does not localise fermentation.
- Because transit speed varies between occasions, a changed curve is consistent with changed transit as well as with changed fermentation.
